HealthTeam Clinic Achieves PCMH Designation

 

Patient Centered Medical Home (PCMH) is a model of healthcare designed to offer patients a primary care team that will provide them coordination of care and assistance with navigating the somewhat segmented heath care system. The principles of a PCMH include a primary care physician led medical team providing comprehensive, coordinated, high quality, safe care.

Blue Cross Blue Shield of Michigan (BCBSM) has a PCMH Designation Program as a component of their Physician Group Incentive Program (PGIP). Participating practices collaborate in development and implementation of PCMH-related tools and processes. The designation program recognizes and supports the primary care practices that have made the most progress in implementing and using patient-centered processes and tools, resulting in delivery of more coordinated, efficient, and effective health care.

In 2008, MSUHT submitted a preliminary list of practice units they wished to nominate for PCMH designation. BCBSM analyzed our self-reported PCMH capabilities and BCBSM claims data from 4th Quarter 2007 through 3rd Quarter 2008 to determine an overall score and ranking. Validation visits were conducted for BCBSM nominated practices. For the 2009 PCMH Designation program, approximately 1,250 PCPs in the highest-scoring practices were awarded the PCMH designation. MSUHT Internal Medicine clinic achieved this significant designation. PCMH Designated practices receive 10% higher reimbursement for Evaluation & Management services for the next year.
